Enrollment Checklist

You will need to take the following steps before we can review your application and schedule an appointment:
    1. Fill out an application
    2. Fill out a financial contract(both parent/guardian signatures are required)
    3. Registration fee submitted?
    4. Enclose a copy of birth certificate
    5. Enclose a copy of the Tennessee School Immunization Certificate (Green Card)
    6. Enclose copy of current standardized test scores
    7. Enclose a copy of current report card or withdrawal form (K5-12th)
    8. Enclose an unofficial copy of student's transcript (10th-12th)
    9. Physical within 12 months prior to the first day of school (K5 only)
    10. Fill out an Emergency Medical Release form (must be notarized)
    11. Fill out an After School Care Registration form
    12. Student Recommendation Form completed by a teacher, pastor, employer etc. (1st thru 12th)
    13. Kindergarten Profile completed by a day care/pre-school employee, etc.
    14. Readiness Screening (K5 only, the office will call to schedule this)
    15. Schedule and complete achievement testing
    16. Schedule an interview with a principal

   To make the application process expedient, please make sure that all of the information listed above is returned to the school at the same time. Our open enrollment will start on March 2nd, at which time we will begin accepting new students for the upcoming school year.

*State law requires students to be five years old by September 30 th to enter kindergarten
